Student Work Permits

14–15 Age Certificate

• Section A would be completed by the Minor’s Prospective Employer.
• Section B would then be completed by the Minor’s Parent or Legal Guardian.
• Certificates must be accompanied with a copy of the minor’s Birth Certificate
• Once Sections A & B have been completed, submit the form & Birth Certificate to jgreene@westvaacademy.org and Sections C will be completed by WVVA and provide copies for the family and the employer and email a copy to the West Virginia Division of Labor (WageandHour@wv.gov) for certification by the Commissioner of Labor.

16–17 Age Certificate

• WVVA no longer has a part in the age verification of 16-17 year old minors.  
• Section A of the certificate should be completed by the student/employer and/or the minor’s parent or guardian.  
• Certificates must be accompanied with a copy of the minor’s Birth Certificate
• Once Section A has been completed, submit the form & Birth Certificate to West Virginia Division of Labor (WageandHour@wv.gov) for processing of Section B.
